Inadequate training came in at the number-two spot for both worker and management representatives, again showing consensus among the group. The third most significant factor was inadequate supervision. "There was at least 80% alignment on the top ten factors. The fact that worker and management representatives largely agreed shows that they are in sync on many key points," says Lok.
